Benjamin Springer, RN, CCM is a Nurse Manager in Ambulatory Care Management with a diverse background spanning inpatient, home care and veteran-focused healthcare systems. With progressive leadership experience, Benjamin has served as a Senior Lead Care Manager and Care Manager, where he specialized in coordinating complex patient care across settings and improving transitions from hospital to home.

His clinical foundation includes work as an RN in both hospital and home care environments, giving him a well-rounded perspective on patient needs across the continuum. In addition, his experience within the Veterans Affairs system has shaped his commitment to serving vulnerable populations and addressing gaps in access, resources and long-term support.

Benjamin is passionate about care coordination, interdisciplinary collaboration and building systems that support patients beyond the clinical setting. He brings a practical, frontline-informed approach to leadership, focusing on improving outcomes for patients with chronic conditions such as heart failure while supporting care teams in delivering high-quality, patient-centered care.
