Adverse Childhood Experiences (ACEs) are strongly associated with chronic disease, behavioral health conditions, treatment nonadherence and high healthcare utilization. Yet many case managers have not received formal training on how childhood trauma influences adult health or how to safely incorporate ACE screening into the case management process. Without this understanding, complex patient needs may be addressed at the surface level while underlying drivers remain unrecognized.

This session addresses the gap between emerging trauma science and practical case management application. Grounded in the landmark ACE study and current research on toxic stress, the presentation explains how early adversity becomes biologically embedded, altering stress response systems, immune function and long-term health risk. Understanding this connection reframes challenging behaviors and repeated utilization patterns through a trauma informed lens.

Participants will be introduced to a practical framework for integrating ACE screening into case management workflows in a manner that prioritizes psychological safety and patient choice. The session will outline strategies for responding to elevated ACE scores without retraumatization, strengthening therapeutic alliance and incorporating resilience building approaches into care plans. Tools for identifying protective factors, engaging community resources and applying trauma informed communication techniques will also be discussed. By connecting the science of adversity with concrete strategies for screening and support, this session equips case managers to move beyond symptom management and toward whole person, trauma informed care that addresses root causes while promoting healing and stability.
